People across Oxfordshire are being invited to attend a community event designed to help them understand the importance of planning ahead for future health and care decisions while continuing to live well today.

The event is a chance for families to find out how they can support themselves and their loved ones navigate planning for future health decisions.

Let’s Talk About Today, Let’s Plan for Tomorrow community event will take place on Thursday 10 September from 1pm to 3pm at Kennington Village Hall, Kennington Road, Kennington, Oxford OX1 5PG.

It is a chance for members of the public to talk with healthcare professionals, other people who have had the same experience, carers, community groups and members of the public to explore the benefits of Advance Care Planning and help families plan for their loved one’s futures.

Palliative Care Week

Our Palliative Care Team is talking to BBC Radio Oxford  exploring palliative care in a special series airing every day this week at 12.10pm. The programmes aim to encourage open conversations about planning ahead, caring for loved ones, and accessing support when it is needed most.

Monday 7th September | Planning
Starting the conversation about future care, including advance care planning and practical steps to help people get started.

Tuesday 8th September | Signs of Dying
Understanding the signs that someone may be deteriorating, including reduced appetite, weight loss, declining mobility and repeated hospital admissions.

Wednesday 9th September | What is Palliative Care?
An introduction to palliative care, when it may be appropriate, how it works and the support it can provide.

Thursday 10th September | Carers’ Support
Information for carers, including Continuing Healthcare (CHC), funding and the support available to help families and carers.

Friday 11th September | Bereavement
Exploring the support and services available to people following the death of a loved one.

Listen live at 12.10pm each day and use the series as a starting point for important conversations about palliative and end-of-life care.

Palliative Anticipatory Care Specialist Joanne Thomas said;

“Advance Care Planning is not about planning for death; it’s about planning for what matters most in life. Completing an Advance Care Plan gives people a voice in their future care, ensuring their wishes, values and preferences are understood and respected.”

Advance Care Planning is a process gives people a supporting framework so that adults at any age or stage of health can understand and share their personal values, life goals, and preferences regarding future medical care. The goal is to help ensure people receive care that is consistent with their values, goals and preferences during serious and chronic illness when they may no longer be able to communicate their preferences.

The free event aims to increase awareness and understanding of:

  • Why Advance Care Planning matters to everyone
  • Patient experiences
  • Making your wishes known
  • Local support and resources available

Attendees will hear from people with first hand knowledge, insights, and understanding through their own personal life experiences, learn why Advance Care Planning matters to everyone, and receive practical information about ways to make their wishes known.

  • Members of the public
  • Carers
  • People living with long-term conditions
  • Healthcare professionals
  • Professionals from community and voluntary sector organisations
