GP and healthcare professionals referrals
We accept referrals from professionals, including mental health professionals, midwives, health visitors and GPs.
The easiest way to refer is by using our secure Referrals Virtual Assistant.
Other ways to refer
If you cannot use the Virtual Assistant:
- You can download a Word version of the referral form
- You can print and complete it if needed
Please complete the referral form and send it to bperinatal.referrals@oxfordhealth.nhs.uk
The referrals inbox is monitored from 9am to 4pm, Monday to Friday (excluding bank holidays).
If you need to make a referral outside these hours, please call the Whiteleaf Centre on 01865 902 000.
After submitting your referral, please call the Perinatal Duty Clinician on 01865 901 749 to discuss the referral and confirm that it has been received.
