Referrals

Referral criteria

We offer support to people with eating disorders who are registered with a GP in Wiltshire. We currently accept referrals from GPs and other professional mental health services. Unfortunately, at this time we do not accept self-referrals.

Signs that someone may need a referral to our service include (but are not limited to):

  • Preoccupation with food and eating
  • Preoccupation with body shape and/or weight
  • Restricted eating:
    • Significantly limiting the quantity of food eaten
    • Significantly limiting the types of food eaten
  • Binge-eating
  • Purging behaviours such as:
    • Excessively exercising
    • Self-inducing vomit
    • Laxative misuse
    • Diuretic misuse
    • Diet Pills
  • Rapid weight loss

Acceptance of referrals is NOT based on Body Mass Index.

Referral form

You can access our referral form below. Please include the following details:

  • Details of eating disordered behaviour
  • Details of any previous treatment
  • Results of most recent blood test
  • Current care plan
  • Weight history
  • Current Body Mass Index (or weight and height)

We ask for weight information to help us understand the level of physical risk. We will not use this to inform whether the person is accepted to the service.
