You will need to start again from the beginning.<\/p>\n<\/div>\n<h2>1. Open the Virtual Assistant<\/h2>\n<p>Click the <strong>Make a Referral<\/strong>\u00a0button located at the bottom-right corner of the screen.<\/p>\n<h2>2. Confirm your identity<\/h2>\n<p>To proceed, you&#8217;ll need to provide some details to verify your identity.<\/p>\n<h2>3. Provide your NHS Number<\/h2>\n<p>Your NHS Number can be found on your appointment letter.<\/p>\n<p>If you have it handy, please enter it when prompted.<\/p>\n<p>Don&#8217;t have your NHS Number? No problem! Select &#8216;No&#8217;, and you&#8217;ll be asked to confirm your <strong>Surname<\/strong> and <strong>Postcode<\/strong> instead.<\/p>\n<div class=\"ohft-box red\">\n<p>If verification fails, you won&#8217;t be able to use the service.<\/p>\n<\/div>\n<h2>4. Provide your Date of Birth<\/h2>\n<p>Enter your <strong>Date of Birth<\/strong> in the required format:<\/p>\n<ul>\n<li>DD\/MM\/YYYY<\/li>\n<li>DD-MM-YYYY<\/li>\n<\/ul>\n<div class=\"ohft-box red\">\n<p>If your NHS Number and Date of Birth don&#8217;t match, you won&#8217;t be able to proceed and will need to contact us directly. See <a href=\"https:\/\/oxfordhealth.nhs.uk\/sun-referrals\/contact\/\">Get in touch<\/a>.<\/p>\n<\/div>\n<h2>5.
