Oxford Health NHS Foundation Trust welcomes the publication of the independent report published by NHS England today, which has highlighted a number of issues for the trust to review.

This independent report follows our own internal investigation into the circumstances leading to Mr Skrebowski’s tragic death on 7 December 2015, for which we continue to offer his family our most heartfelt condolences.

The trust fully accepts all the recommendations made in the report and has already taken action to ensure it has implemented the majority of these prior to today’s publication.

While the report is clear this tragic incident could not have been prevented, we acknowledge there were things we could have done better and for that we are sorry.
