Chapter 9 looks at safety strategies in primary care, while chapter 11 is a compendium of safety strategies and interventions.<\/span><\/p>\n<p>The <a href=\"https:\/\/www.patientsafetyacademy.co.uk\/training\" target=\"_blank\" rel=\"noopener noreferrer\">Patient Safety Academy<\/a> offers a number of training courses commissioned by HEVTV including<\/p>\n<ul>\n<li><a href=\"https:\/\/www.patientsafetyacademy.co.uk\/half-day-course\" target=\"_blank\" rel=\"noopener noreferrer\">Introduction to\u00a0Human Factors\/Ergonomics<\/a>\u00a0(half day)<\/li>\n<li class=\"font_7\"><a href=\"https:\/\/www.patientsafetyacademy.co.uk\/analysis-of-clinical-incidents\" target=\"_blank\" rel=\"noopener noreferrer\">Analysis of Clinical Incidents<\/a>\u00a0(two days)<\/li>\n<li class=\"font_7\"><a href=\"https:\/\/www.patientsafetyacademy.co.uk\/managing-qi\" target=\"_blank\" rel=\"noopener noreferrer\">Leading an\u00a0Improvement Project<\/a>\u00a0(two days)<\/li>\n<li class=\"font_7\"><a href=\"https:\/\/www.patientsafetyacademy.co.uk\/train-the-trainer\" target=\"_blank\" rel=\"noopener noreferrer\">Train the Trainer<\/a>\u00a0(two days)<\/li>\n<\/ul>\n<p>The National Patient Safety Agency (NPSA) was transferred to\u00a0<a href=\"https:\/\/improvement.nhs.uk\/resources\/learning-from-patient-safety-incidents\/\" target=\"_blank\" rel=\"noopener noreferrer\">NHS Improvement<\/a>\u00a0in 2016.<\/p>\n<div class=\"container\">\n<details class=\"nhsuk-details nhsuk-expander\">\n                    <summary class=\"nhsuk-details__summary\">\n                        <span class=\"nhsuk-details__summary-text\">Promoting patient safety in general practice<\/span>\n                    <\/summary>\n                    <div class=\"nhsuk-details__text\">\n<p>The <a href=\"https:\/\/www.e-lfh.org.uk\/programmes\/freedom-to-speak-up\/\">Freedom to Speak Up in Healthcare in England programme<\/a> to support NHS staff who want to raise concerns on issues such as patient safety is being updated <span data-contrast=\"none\">during\u00a0national <a href=\"https:\/\/www.nationalguardian.org.uk\/speak-up-month-2020\/\" target=\"_blank\" rel=\"noopener noreferrer\">Speak up Month 2020.<\/a><\/span><\/p>\n<p><a href=\"https:\/\/www.health.org.uk\/publication\/improving-safety-primary-care\" target=\"_blank\" rel=\"noopener noreferrer\">Improving safety in primary care:<\/a> The Health Foundation reports that the strategies showing promise target the key causes of harm in primary care: clinical complexity (via computerised prescribing and alert systems), human factors (via pharmacist input) and systems issues (using learning collaboratives, audit, feedback and discharge planning to improve interfaces with secondary care).<\/p>\n<p>The BMJ Patient Safety is resource centre for NHS staff including research, national standards, implementation guidelines and case studies to help ensure patients always receive safe and reliable care.
