SCIE offers resources on a wide range of topics, including practical tools to support dignity in care, equipment and telecare, medication, mental health issues (particularly dementia), nutrition plus materials for staff working in care homes.<\/p>\n<p><a href=\"https:\/\/fusion48.net\/frailty\" target=\"_blank\" rel=\"noopener noreferrer\">Frailty360<\/a> is a frailty training programme that uses a unique set of\u00a0\u00a0resources and an immersive approach to education to deliver key learning\u00a0objectives in frailty and integrated person-centred care. It includes the<\/p>\n<ul>\n<li><a href=\"https:\/\/www.frailtytoolkit.org\/category\/about-the-toolkit\/\" target=\"_blank\" rel=\"noopener noreferrer\">Frailty Toolkit<\/a> &#8211; a range of local resources and case studies of real examples of supported self-care in action and<\/li>\n<li><a href=\"https:\/\/www.frailtytoolkit.org\/frailtysim-vr\/\" target=\"_blank\" rel=\"noopener noreferrer\">FrailtySIM<\/a> &#8211; a Virtual Reality workforce development tool that improves healthcare for older people living with frailty which provides the user with the opportunity to\u00a0\u201cwalk\u201d in the shoes of an older person living with frailty.<\/li>\n<\/ul>\n<p>The British Geriatric Society has published a\u00a0<span lang=\"EN-US\"><span lang=\"EN-GB\"><a href=\"https:\/\/www.bgs.org.uk\/resources\/resource-series\/comprehensive-geriatric-assessment-toolkit-for-primary-care-practitioners\" target=\"_blank\" rel=\"noopener noreferrer\">Comprehensive Geriatric Assessment Toolkit for Primary Care Practitioners<\/a><\/span><\/span>. The CGA toolkit explains what comprehensive geriatric assessment is, in what circumstances to use it and how it is done together with planning and involvement of social services.<\/p>\n<p>Public Health England (PHE) and the National Centre for Sport and Exercise Medicine have developed an <a href=\"https:\/\/www.ncsem-em.org.uk\/research\/ncsem-em-launches-older-adults-resource-with-public-health-england\/\" target=\"_blank\" rel=\"noopener noreferrer\">e-learning module<\/a> to provide information about the benefits of physical activity for older adults and in particular those who are beginning to experience a decline in their physical function, referred to as those who are \u2018in transition\u2019. Older adults \u2018in transition\u2019 are those mainly over the age of 65 whose function is declining due to low levels of activity and too much sedentary time. They may have lost muscle strength and\/or are overweight, but otherwise remain reasonably healthy.
