The <a href=\"https:\/\/uclpartners.com\/long-term-condition-support\/\" target=\"_blank\" rel=\"noopener noreferrer\">long-term condition framework and support package <\/a>\u00a0focuses on asthma and COPD, and is based on four core principles:<\/p>\n<ul>\n<li>Virtual by default<\/li>\n<li>Mobilising the wider workforce (including pharmacists, HCAs, nursing associates, social prescribers and others)<\/li>\n<li>Step change in support for self-management<\/li>\n<li>Digital innovation, including tools for self-management and technology for remote monitoring.<\/li>\n<\/ul>\n<p>It includes search and risk stratification tools, education and training resources and digital tools.<\/p>\n<hr \/>\n<p class=\"NoSpacing\" style=\"color: #444444\"><span style=\"font-weight: inherit\">Read about the some of the resources the <strong>Royal College of General Practitioners<\/strong> has produced relating to long-term conditions <a href=\"https:\/\/www.rcgp.org.uk\/policy\/rcgp-policy-areas\/long-term-conditions.aspx\" target=\"_blank\" rel=\"noopener noreferrer\">here<\/a>, including their <a href=\"https:\/\/www.rcgp.org.uk\/policy\/fit-for-the-future\/our-vision.aspx\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: inherit\">2022 Vision<\/span>\u00a0for the future of general practice.<\/a><\/span><\/p>\n<p><strong>Learning Resources<\/strong><\/p>\n<p><a href=\"https:\/\/www.educationforhealth.org\/\" target=\"_blank\" rel=\"noopener noreferrer\">Education for Health<\/a> aim to support the development of the knowledge, values and competencies required to manage Long Term Conditions.\u00a0They offer a range of level 5, 6, 7 and 8 courses, including a number of <a href=\"https:\/\/www.educationforhealth.org\/allresources\/free-elearning\/\" target=\"_blank\" rel=\"noopener noreferrer\">free elearning resources<\/a><\/p>\n<p><strong>Care planning<\/strong><\/p>\n<p>Care planning is all about confidence, forging an interactive partnership between clinician and patient to support self management of their long term condition by patients.\u00a0\u00a0Read about the RCGP&#8217;s work around\u00a0<a href=\"https:\/\/www.rcgp.org.uk\/clinical-and-research\/resources\/toolkits\/collaborative-care-and-support-planning-toolkit.aspx\" target=\"_blank\" rel=\"noopener noreferrer\">Collaborative Care and Support Planning.<\/a><\/p>\n<ul>\n<li>Thames Valley Strategic Clinical Network are promoting the use of <a href=\"https:\/\/tvscn.nhs.uk\/networks\/long-term-conditions\/care-planning-patients-experts-managing-conditions\/\" target=\"_blank\" rel=\"noopener noreferrer\">Patient-centred Care Planning<\/a> \u00a0through training across TV<\/li>\n<li>Read about the work of the <a href=\"https:\/\/coalitionforcollaborativecare.org.uk\/\" target=\"_blank\" rel=\"noopener noreferrer\">Coalition for Collaborative Care<\/a> (C4CC) and how it is working to make person-centred, collaborative care for people with LTCs the norm.<\/li>\n<\/ul>\n<h4><strong>Clinical conditions:<\/strong><\/h4>\n<details class=\"nhsuk-details nhsuk-expander\">\n                    <summary class=\"nhsuk-details__summary\">\n                        <span class=\"nhsuk-details__summary-text\">Deafness and Hearing Loss<\/span>\n                    <\/summary>\n                    <div class=\"nhsuk-details__text\">\n<p>RCGP have developed a\u00a0<a href=\"https:\/\/www.rcgp.org.uk\/clinical-and-research\/resources\/toolkits\/deafness-and-hearing-loss-toolkit.aspx\" target=\"_blank\" rel=\"noopener noreferrer\">Deafness and Hearing Loss toolkit<\/a> which supports GPs and GP trainees to implement the latest NICE guidelines and NHS Accessibility Quality Standard and guidance across the UK. The resources developed aim to educate GPs and trainees on deafness and hearing loss, help reduce variations in accessibility to GP practices and ensure deafness and hearing loss are considered across all aspects of primary care activity including consultations and continued care.<\/p>\n<\/div>\n                <\/details>\n<details class=\"nhsuk-details nhsuk-expander\">\n                    <summary class=\"nhsuk-details__summary\">\n                        <span class=\"nhsuk-details__summary-text\">Dementia <\/span>\n                    <\/summary>\n                    <div class=\"nhsuk-details__text\">\n<p><strong>See <a href=\"https:\/\/oxfordhealth.nhs.uk\/library\/cpd-zone\/cpd-by-topic\/dementia-care\/\" target=\"_blank\" rel=\"noopener noreferrer\">Dementia Care<\/a><\/strong><\/p>\n<\/div>\n                <\/details>\n<details class=\"nhsuk-details nhsuk-expander\">\n                    <summary class=\"nhsuk-details__summary\">\n                        <span class=\"nhsuk-details__summary-text\">Diabetes<\/span>\n                    <\/summary>\n                    <div class=\"nhsuk-details__text\">\n<p>There are a range of course on diabetes available on the<a href=\"https:\/\/elearning.rcgp.org.uk\/course\/search.php?search=diabetes\" target=\"_blank\" rel=\"noopener noreferrer\"> RCGP Learning platform<\/a>, including <a class=\"\" href=\"https:\/\/elearning.rcgp.org.uk\/course\/search.php?search=dpp\" target=\"_blank\" rel=\"noopener noreferrer\">Non-diabetic Hyperglycaemia and the\u00a0NHS Diabetes Prevention Programme<\/a>\u00a0which discuss the evidence behind the programme\u00a0and who should be offered referral.<\/p>\n<p>Diabetes UK has produced <a href=\"https:\/\/www.diabetesinhealthcare.co.uk\/lnt\/Login.aspx\" target=\"_blank\" rel=\"noopener noreferrer\">Diabetes in Healthcare<\/a>, an introductory course accredited by the Royal College of Nursing for healthcare professionals who are not specialists in diabetes but want to know more about the condition.<\/p>\n<p>Other diabetes CPD resources include<\/p>\n<ul>\n<li><a href=\"https:\/\/store.educationforhealth.org\/catalog?pagename=Diabetes\" target=\"_blank\" rel=\"noopener noreferrer\">Education for Health<\/a> level 5 and 6 Diabetes\u00a0 courses.<\/li>\n<li class=\" subtitles\"><a href=\"https:\/\/www.cdep.org.uk\/\" target=\"_blank\" rel=\"noopener noreferrer\">Cambridge Diabetes Education Programme<\/a><\/li>\n<li><a href=\"https:\/\/www.diabetesonthenet.com\/course\" target=\"_blank\" rel=\"noopener noreferrer\">Diabetes on the Net<\/a><\/li>\n<li><a href=\"https:\/\/www.pcdsociety.org\/course\" target=\"_blank\" rel=\"noopener\">Primary Care Diabetes Society<\/a> Training courses for primary care<\/li>\n<\/ul>\n<p>In an article in GM Journal, the authors discuss\u00a0\u00a0<a href=\"https:\/\/www.gmjournal.co.uk\/the-challenge-of-managing-type-1-diabetes-in-the-older-patient\" target=\"_blank\" rel=\"noopener noreferrer\">The challenge of managing type 2 diabetes in the older patient.<\/a><\/p>\n<\/div>\n                <\/details>\n<details class=\"nhsuk-details nhsuk-expander\">\n                    <summary class=\"nhsuk-details__summary\">\n                        <span class=\"nhsuk-details__summary-text\">Epilepsy<\/span>\n                    <\/summary>\n                    <div class=\"nhsuk-details__text\">\n<p>There are two elearning packages on <a href=\"https:\/\/www.e-lfh.org.uk\/programmes\/epilepsy\/\" target=\"_blank\" rel=\"noopener noreferrer\">Epilepsy<\/a> on the e-learning on the eLfH platform. One is designed to ensure that GPs have access to a high quality\u00a0learning module to enable better support for people living with epilepsy and to raise awareness of the whole-person approach to care, resulting in reduced risk of repeat acute admissions and unnecessary deaths within the epilepsy population.\u00a0 The second will enable healthcare professionals, particularly those who are not specialists in Epilepsy, to better understand Sudden Unexpected Death in Epilepsy (SUDEP).<\/p>\n<\/div>\n                <\/details>\n<details class=\"nhsuk-details nhsuk-expander\">\n                    <summary class=\"nhsuk-details__summary\">\n                        <span class=\"nhsuk-details__summary-text\">Musculoskeletal  disorders<\/span>\n                    <\/summary>\n                    <div class=\"nhsuk-details__text\">\n<p>The <a href=\"https:\/\/www.e-lfh.org.uk\/programmes\/musculoskeletal-primary-care\/\" target=\"_blank\" rel=\"noopener noreferrer\">Musculoskeletal primary care e-learning programme<\/a> has been developed to support clinicians working in primary care as first contact practitioners (FCP)\u00a0 in a primary care environment.<\/p>\n<p>Working with the Royal College of General Practitioners (RCGP) the Scoliosis Association UK have developed an <a href=\"https:\/\/elearning.rcgp.org.uk\/course\/info.php?id=405\" target=\"_blank\" rel=\"noopener noreferrer\">e-learning module focussed on scoliosis<\/a>\u00a0which aims to help GPs to assess patients with scoliosis, recognise who needs referral and in what time frame.
