What is the Care Home Support Service?

The Care Home Support Service (CHSS) is a multidisciplinary team. It consists of specialist nurses, physiotherapists, occupational therapists and healthcare assistants for both physical and mental health disciplines.

A medical consultant and psychologist support the clinical leadership to the team and administrative support is provided by a small team.

The CHSS – supports, signposts, educates and advises care home staff to promote a high standard of care for residents in Oxfordshire residential and nursing homes.

What do we do

The Care Home Support Service Team (CHSS) works in partnership with care home staff across Oxfordshire to support the care of residents.

Physical Health Nurse team

• Assessment /screen of all new residents
• Falls assessments
• Supporting residents, families, and carers in decisions about end of life
• Exclusion of physical health causes for distressed behaviour in dementia and supporting with wellbeing interventions

Therapy team

• Manual handling advice
• Mobility assessments
• Postural management advice
• Seating assessments and advice
• Any other therapy interventions other than longer term rehab

Mental health team

• Helping with the management of residents with dementia whose behaviour is challenging
• Working primarily with care staff helping them implement psycho-social interventions
• The CHSS team will work closely with professionals in other existing health care services especially the community mental health teams

Each resident’s own GP will retain medical responsibility at all times with the team providing additional nursing, therapy and mental health support where appropriate.
