The vaccination may then be delayed and an appointment at a community clinic may be required.<\/p>\n<p>If you need any support completing a consent form, please email us at <a href=\"mailto:immunisationteam@oxfordhealth.nhs.uk\">immunisationteam@oxfordhealth.nhs.uk<\/a> providing your name and telephone number. We shall then be in contact to help you.<\/p>\n<p>Please check that you have the correct Unique School Code.
