\/><\/p>\n<h2>What do we do<\/h2>\n<p><span data-contrast=\"auto\">The care home support service team (CHSS), works in partnership with care home staff across Oxfordshire to support the care of residents. <\/span><\/p>\n<p><span data-contrast=\"auto\">This site will be updated frequently by the clinical team to ensure that the resources provided are up to date and support best practice.<\/span><\/p>\n<p>A physical health nurse will visit your care home each month to undertake planned assessments.<\/p>\n<h2>Our teams<\/h2>\n<details class=\"nhsuk-details nhsuk-expander\">\n                    <div class=\"nhsuk-details__text\">\n<ul>\n<li>Assessment \/screen of all new residents<\/li>\n<li>Falls assessments<\/li>\n<li>Supporting residents, families, and carers in decisions about end of life<\/li>\n<li>Exclusion of physical health causes for distressed behaviour in dementia and supporting with wellbeing interventions<\/li>\n<\/ul>\n<p><a href=\"https:\/\/oxfordhealth.nhs.uk\/wp-content\/uploads\/sites\/24\/2024\/04\/CHSS-referral-form-April-2024.docx\">CHSS referral form <\/a><\/p>\n<\/div>\n                <\/details>\n<details advice<\/li>\n<li>Seating assessments and advice<\/li>\n<li>Any other therapy interventions other than longer term rehab<\/li>\n<\/ul>\n<p><a team<\/span>\n                    <\/summary>\n                    <div class=\"nhsuk-details__text\">\n<ul>\n<li>Helping with the management of residents with dementia whose behaviour is challenging.<\/li>\n<li>Working primarily with care staff helping them implement psycho-social interventions.<\/li>\n<li>The CHSS team will work closely with professionals in other existing health care services especially the community mental health teams.<\/li>\n<\/ul>\n<p><a
