The Autism & Learning Disability Liaison Team (ALDLT) is a team of Care Navigators with specialist knowledge and experience in supporting people with a learning disability, autism, or both.<\/p>\n
Our team helps ensure that children, young people, and their families receive the right support at the right time. We work to make local systems more responsive, ensuring services are coordinated and meet needs in a holistic way.<\/p>\n

We are not a clinical team, and the Care Navigators will not replace existing care or interventions. Our role is to ensure continuity of care for the child or young person by working across systems and services to ensure their needs are met.<\/p>\n
The manager provides the leadership and management for the team. If you have any concerns, you can talk to them.<\/p>\n<\/div>\n <\/details>\n Will work as a Care Navigator but also provide support for the rest of the team.<\/p>\n<\/div>\n <\/details>\n The care navigator will be the main contact with the team and will introduce themselves to you.<\/p>\n<\/div>\n <\/details>\n Supports young people in improving their well-being by connecting them with community resources, services, and activities.<\/p>\n They provide both direct and indirect support, working collaboratively with young people, parents, carers, and professionals.<\/p>\n Their role involves identifying goals, addressing barriers, and creating personalised plans based on what the young person wants to achieve.<\/p>\n<\/div>\n <\/details>\n Provides emotional and practical support to parents and carers, guided by their individual needs and shared lived experience.<\/p>\n They contribute to the family’s professional network with a parent-centred approach, offering alternative insights and encouraging new perspectives.<\/p>\n<\/div>\n <\/details>\n Supports the team with organisational and logistical tasks and may well be the first member of the team you talk to!<\/p>\n<\/div>\n <\/details>\n\n Care Navigator Clinical Lead<\/span>\n <\/summary>\n
