Children and young people in this group experience the highest levels of health inequality in society. You can read more about this definition in this document \u2018<a href=\"https:\/\/www.england.nhs.uk\/commissioning\/health-just\/children-and-young-people\/\">The \u2018Framework for Integrated Care\u2019<\/a> .<\/p>\n<p>These young people&#8217;s needs are often:<\/p>\n<ul>\n<li><strong>Multiple<\/strong> (ie not just in one area, such as mental and physical health)<\/li>\n<li><strong>Persistent<\/strong> (ie long term, including for example learning disability, autism or both)<\/li>\n<li><strong>Severe<\/strong> (ie not responding to standard \u2018clinical\u2019 interventions)<\/li>\n<li><strong>Framed by family and social contexts<\/strong> (such as: early family disruption, loss or inequality, among others<\/li>\n<\/ul>\n<h2>Significant concerns<\/h2>\n<p>Often, professionals and others who provide day-to-day care and support for the young people have significant concerns about the young person\u2019s ongoing emotional, mental health or neurodevelopmental difficulties.<\/p>\n<h2>Referrals to the county-wide link teams<\/h2>\n<p>The young people referred are likely to be those who:<\/p>\n<ul>\n<li><strong>Present with challenging behaviours<\/strong> \u2013 there may have been several referrals to different health, social care and other services. This may be because their presentations don\u2019t \u2018fit\u2019 with existing services, which may only specialise in one of their needs eg autism.<\/li>\n<li><strong>Find engagement difficult<\/strong> \u2013 sometimes services find it difficult or are unable to engage some young people in cross-agency plans.
