Oxfordshire Autism and Learning Disability Liaison Team

The Autism & Learning Disability Liaison Team (ALDLT) is a team of Care Navigators with specialist knowledge and experience in supporting people with a learning disability, autism, or both.

Our team helps ensure that children, young people, and their families receive the right support at the right time. We work to make local systems more responsive, ensuring services are coordinated and meet needs in a holistic way.

Who can access our support?

Young people who:

  • Be an Oxfordshire resident or be a child in care out of county, under the responsibility of the Oxfordshire local authority.
  • Are aged 0-25 years.
  • Have a diagnosis of a Learning Disability, Autism or both.
  • Are at risk of hospital admission, are currently in hospital and/or at risk of placement breakdown.

All of the above would highlight eligibility to be added to the Dynamic Support Register (DSR).

Our role as Care Navigators

We are not a clinical team, and the Care Navigators will not replace existing care or interventions. Our role is to ensure continuity of care for the child or young person by working across systems and services to ensure their needs are met.

How we support you

Helping you navigate the system
  • Be a single point of contact for you and your family.
  • Ensure you are included in and informed about decisions regarding your care.
  • Advocate on your behalf when requested and help communicate your family’s story within the system.
Coordinating support networks
  • Organise and support networks surrounding you and your family, including extended family members, friends, and other supporters.
  • Support the implementation of Specialist Care Plans to help you live in the community.
  • Share key information with those involved in your care to help keep you safe.
Addressing challenges and ensuring progress
  • Identify any unmet needs or gaps in support provision.
  • Remove barriers preventing you from accessing timely and personalised support.
  • Support the Care, Education, and Treatment Review (CETR) process.
  • Ensure that CETR recommendations are completed in a timely manner and that updates are made to the Dynamic Support Register.

Meet the team

Team Manager

The manager provides the leadership and management for the team. If you have any concerns, you can talk to them.

Care Navigator Clinical Lead

Will work as a Care Navigator but also provide support for the rest of the team.

Care Navigators

The care navigator will be the main contact with the team and will introduce themselves to you.

Social Prescriber

Can help signpost to local resources. They might help identify a young person’s interests, which might identify what support would be right for them.

Parent/Carer peer support worker

Can support families through their lived experience, and challenge professionals to think differently!

Administrator

Supports the team with organisational and logistical tasks and may well be the first member of the team you talk to!
